Agent Appointments
If you would like to be considered for an appointment as an agent for Capstone Underwriters, LLC., please complete the Agent Profile form below. Sign and date the form and e-mail it to agent@capstoneunderwriters.com along with a copy of your General Lines License of your state of residence and E&O declarations page. We will then contact you directly regarding the appointment.
Requirements
- Complete signed and dated Agent Profile Form
- Current General Lines License of your state of residence
- Current Errors and Omissions coverage for minimum limit of $500,000
- Current W-9 Form
- Agent’s personal credit in compliance with Capstone’s guidelines
